Per Diem 360 ABA Services Package Deal

First-Time Enrollment

To begin Applied ABA & Early Learning Services through 360 Life Changes, all new clients must purchase this one-time enrollment package before scheduling their initial one-hour assessment session.

Client Renewal Policy

To ensure your child’s individualized learning plan and client records remain accurate and up to date, an updated 360 Life Changes Assessment is required every two (2) years from your child’s original start date for clients who are not actively participating in weekly services.

Weekly Client Attendance Policy

Clients enrolled in Applied ABA & Early Learning Services are expected to attend their scheduled appointments consistently. To remain an active weekly client:

Weekly attendance is required.
One (1) excused absence per calendar month is permitted.
Additional unexcused absences or extended gaps in attendance may result in your child being withdrawn from weekly services and require a new enrollment before services can resume.

Per Diem (As-Needed) Clients

Families who schedule services on an occasional or as-needed (Per Diem) basis must purchase the Per Diem 360 ABA Services Package every two (2) years. This renewal ensures your child’s assessment, educational records, and client file remain current so we can continue providing individualized instruction and support.

After purchasing this package, please select your preferred assessment date and time from the available appointment options below.

Available Assessment Times (Saturdays Only):

During checkout, please include the following information in the Order Notes section:

Preferred appointment date and time (if different from the available time slots listed above)
Participant’s name (if different from the purchaser)
Parent/Guardian or Point of Contact name (if applicable)

Once your order has been received, we will review your requested appointment and send a confirmation by phone call, text message, or email. If your preferred time is unavailable, we will contact you to schedule the next available appointment that best fits your schedule.
