1. 1. Complete the 360 Life Changes ABA Consent & Waiver Form (required before booking)

Important Information

• Forms must be submitted prior to the session.
  Missing forms will result in cancellation and the session fee will be non-refundable.

• Parents may be asked to bring specific materials depending on training focus.

• Capacity is limited:
  Sessions are capped at 6 children, with grouping based on assessment level when possible.

• Off-site requests:
  Any location outside the Lake Worth center must be approved in advance and may require adjusted pricing.
